Designing the Approval Mesh

Human-in-the-loop is easy to describe and hard to build well. The Approval Mesh routes a paused action to the right approver with full context — what the agent wants to do, why, and what it will cost.

The design goal was simple: never make a human guess. Every approval carries its trace, its policy, and its blast radius.
